Demonstrate Knowledge and Skills
I = Independently
The student performs age appropriate cognitive, motor and physical education skills following instruction and practice without additional teacher or student assistance.
For example: Upon the teachers request, the student was able to demonstrate the difference between a skip and gallop; when asked the difference between skipping and galloping, the student was able to explain the difference and perform each skill correctly.
W = With Assistance
The student performs age appropriate cognitive, motor and physical education skills some of the time but not consistently following instruction and practice. Additional teacher input is needed to assist the student to perform a skill consistently.
For example: The student was unable to demonstrate the difference between skipping and galloping independently and consistently (physically and cognitively); the student needs reminders from the teacher.
N = Not Apparent at this time
The student was unable or unwilling to demonstrate the difference between a skip and a gallop upon teacher request, even after additional prompting.

The student is consistent in his/her commitment and responses to tasks given in class:
* consistently participates at a very high level of activity
* consistently is attentive and follows directions
* consistently cooperates with the teacher and other students at age-appropriate levels
2= Satisfactory Effort
The student displays a commitment in responding to tasks given in class most of the time but not always
* participates at a moderate level of activity
* follows directions given in class (visually, verbally), most of the time
* cooperates with the teacher and other students most of the time
3 = Needs Improvement in Effort
The student is inconsistent in his/her commitment and response to tasks given in class
* participates at a low level of activity
* follows directions inconsistently
* experiences difficulty cooperating with classmates and teacher
The student is consistent in his/her commitment and responses to tasks given in class:
* consistently participates at a very high level of activity
* consistently is attentive and follows directions
* consistently cooperates with the teacher and other students at age-appropriate levels
2= Satisfactory Effort

Demonstrates Knowledge and Skill
A = Highest Level
Student is performing physical education skills and tasks consistently at a very high skill level.
B = High Level
Student is performing physical education skills and tasks at a high skill level most of the time.
C = Acceptable Level
Student is performing skills and tasks at an age-appropriate level. Additional practice could be helpful in raising performance level.
D = Low Level
Student is inconsistent in his/her performance of skills and tasks and inconsistent in performing skills and tasks at an age appropriate level.
E = Unsatisfactory Level
Student is not completing age appropriate skills and tasks.
Student is performing physical education skills and tasks consistently at a very high skill level.
B = High Level
Student is performing physical education skills and tasks at a high skill level most of the time.
C = Acceptable Level
Student is performing skills and tasks at an age-appropriate level. Additional practice could be helpful in raising performance level.
D = Low Level
Student is inconsistent in his/her performance of skills and tasks and inconsistent in performing skills and tasks at an age appropriate level.
E = Unsatisfactory Level
Student is not completing age appropriate skills and tasks.
Effort
1 = Outstanding Effort
The student consistently
* shows positive attitude in Physical Education
* exhibits task commitment
- consistently responds appropriately to whatever is asked upon initial request
- consistently persistent when working on tasks
- takes appropriate risks
* Participates at a very high activity level and cooperates with teacher and classmates
* puts forth best attempt in quality and quantity of work
* completes classwork/homework assignments on time
2 = Satisfactory Effort
The student most of the time
* shows positive attitude in Physical Education
* exhibits task commitment in responding to whatever is asked
* participates at a moderate to high level of activity
* puts forth best attempt (most of the time but not consistently)
* completes classwork/homework assignments on time
3 = Needs Improvement in Effort
The student is inconsistent in the following areas:
* positive attitude - is not displayed consistently
* task commitment:
- responding inappropriately to whatever asked
- is not persistent in completing tasks (gives up easily)
- does not take appropriate risks
* participates at a low activity level
* best attempt (quality and quantity of work is not performed) is not given
* classwork/homework assignments are not completed thoroughly on time
